The Solution
HealthSmart currently uses LexisNexis® Population Health Monitor, a robust risk stratification solution leveraging MEDai Science™ technology to deliver models that proactively identify risks impacting health outcomes. To further improve their population health management program, HealthSmart ran a test trial of the Motivation Index, a predictive model combining claims, pharmacy, and medical data. This model identifies members posing the highest health risks and those most likely to engage in health improvement programs. The trial included 104 impactable members with specific health conditions and risk levels, aiming to enhance engagement in care plans.
Operational Impact
  • HealthSmart nurses made phone calls to individual members identified through the company’s disease management program.
  • The goal was to get as many individuals as possible to complete a health assessment and enroll in a customized care plan.
